Dementia Care Including Alzheimer's Assistance
All staff are trained and certified to provide assistance to individuals diagnosed with dementia, including Alzheimer’s disease. Care Plans are customized to include assistance with cueing, reassurance and redirection while avoiding any potential triggers for anxiety. Additionally, Care Plans are continuously monitored and updated to reflect any changes in condition.
Hospice Assistance/Comfort Measures
During this important time, our staff is committed to providing compassionate assistance to both our clients and their families. We will work in conjunction with your preferred Hospice provider to make sure that your loved one is clean and comfortable at all times, while monitoring for pain symptoms and changes in condition. We will consistently perform the necessary duties to ensure that that your primary focus is on spending time together and not daily tasks associated with care.
Hospital/Facility Discharge Assistance
Our team of caring professionals is available 24/7 to assist with transitions home. Services may be initiated on short-notice with the option of transportation assistance if needed. Upon home arrival, we will assist with shopping or cleaning as needed. Additionally, Care Managers will provide complimentary Home Safety Evaluations to make sure the overall environment is safe and organized for your loved one’s continued recovery.
Long-Term Care Insurance Benefit Verification And Claims Service
With over 15 years experience working with Long-Term Care Insurance, our team is prepared to assist you in the both the claims and billing/reimbursement process. We will provide a free benefits verification consultation in addition to managing the requests of your insurance provider. You will not be responsible for submitting paperwork, nor gaining claim approval.
Placement Assistance
Free Home Safety Evaluations
For many of our clients, it is the first time they are receiving in-home care. Because of this, we are happy to use our experience to offer free in-home safety evaluations. We will help you identify simple, cost-effective home modifications that will promote the safety and independence of your loved one.
